In order to streamline the paperwork process, we have provided the forms that we will need to begin your treatment with our office. Please feel free to download these forms and complete them prior to your first appointment. You may either bring them with you to your initial visit or fax them to our office at 719-955-1259. This will reduce the time spent completing this paperwork in our waiting area and allow greater time with your provider and our clinical staff.
